
PATIENT ENROLLMENT
As a patient of Dundas Family Medical Group, you may be asked to sign a Ministry of Health enrolment form. This form allows you to become a rostered patient within our Family Health Organization.
Being a rostered patient provides you with access to a range of services, including nutrition support, social work, and psychiatry.
By signing the enrolment form, you agree to contact our office for your healthcare needs and to use the Telephone Health Advisory Service for after-hours care.
Please note that if you seek care outside of our clinic (such as walk-in clinics, after-hours clinics, or other family physicians), the physicians at Dundas Family Medical Group may be billed for those services. For this reason, it is very important that you contact our office or the Telephone Health Advisory Service whenever possible for your healthcare needs.

It is extremely important that you keep our office updated with your most current email address.
Our team uses secure email to communicate important information such as test results, specialist appointments, appointment reminders, and more.
If your email address has changed or you are unsure what we have on file, please contact our office to ensure your information is up to date.
Allied Health Professionals
As a patient of the Dundas Family Medical Group you have access to a social worker, dietitian, pharmacist, respiratory nurse educator and a psychiatrist. If you feel that you would benefit by seeing one of these professionals, please speak to your family doctor and he/she can assist you with a referral.
