As a patient of the Dundas Family Medical Group, you will be/were asked to sign a form for the Ministry of Health. This form is an enrolment form, which allows you to become a rostered patient. Becoming a rostered patient gives you access to services provided by the Family Health Organization such as nutrition, social work and psychiatry. By signing the roster form, you agree to call our office for your healthcare needs and the Telephone Health Advisory Service for after hours care. The physicians of the Dundas Family Medical Group will be charged 100% of the cost for any outside services that you use i.e. walk in clinics, after hours clinics or other family doctors. It is therefore extremely important that you contact our office/The Telephone Health Advisory Service for your healthcare needs.
Prescriptions
Some of our physicians do not renew prescriptions by phone or fax. Please make sure that you call to book an appointment at least 3 weeks before you are due to run out of your medications. This will allow plenty of time to get an appointment with your physician. Booking an appointment with your physician for prescription renewals allows your physician to better track the effectiveness of the medication and to prevent any complications from occurring. If you have forgotten to book an appointment and your prescription has run out, there will be a $15.00 charge for renewing it without an appointment. You will be given a prescription for enough medication to get you to your appointment.
Test Results
We do not give test results by phone unless your physician has specifically indicated that this will be done. If you are concerned about the results of a test, please book an appointment to see your physician. If there is a problem with your test results a nurse /physician will contact you. If you require a copy of test results there will be a charge depending on the number of pages required. Please check with the receptionist for the amounts.
Forms
If you have forms for your physician to complete please make sure that all your pertinent information is filled out before dropping them off. It is especially important that the consent to release information is signed. If this portion is not filled out or signed we cannot release the information.
Forms cannot be done while you wait. You will be contacted when the form is ready for pickup and you will be notified of any charges at that time.
Allied Health Professionals
As a patient of the Dundas Family Medical Group you have access to a social worker, dietitian, pharmacist, respiratory nurse educator and a psychiatrist. If you feel that you would benefit by seeing one of these professionals, please speak to your family doctor and he/she can assist you with a referral.
